This patient in her 30s was 5’3” tall, weighed 135lb and wore a 34B – 32C bra preoperatively (bra sizes will vary depending on the manufacturer, which is why I never guarantee a specific cup size after breast augmentation surgery). She wanted to regain the breast volume and fullness that she lost after having children. Her left breast was significantly larger with a slightly lower nipple position and there was bilateral stage I ptosis. We discussed the advantages and disadvantages of having a subfascial breast implant pocket versus dual plane pocket. The subfascial pocket does not provide as much tissue coverage over the implants, but there is no risk of pectoralis muscle animation or functional changes. Since she had good tissue thickness with no visible ribs on the chest, so she decided to have subfascial pockets. She chose a 445cc implant for the smaller right side, then I used sterile implant sizers during surgery and found that a 375cc implant provided the best overall improvement in shape and symmetry. Note that inserting breast implants never corrects a pre-existing difference in nipple position, so the left nipple-areola remained slightly lower. Nonetheless a very good, natural-appearing breast augmentation result was achieved. The postoperative pictures were taken one year later.
