This patient in her early 20s had underwent urgent abdominal surgery in her late teens, resulting in a midline scar above the belly button. In order to get rid of this scar she had a tummy tuck elsewhere, but was unhappy with the result. The scar was high and irregular on the lower abdomen, with standing cones of tissue (dog ears) at each end. She also felt that the pubic mound was pulled abnormally high and tight. The plan was to perform a revision mini tummy tuck with abdominal liposuction, which would help mobilize any remaining skin and facilitate a lower to final scar position. In addition, during wound closure the elevated tissue on the abdomen was anchored to the muscles on the lower abdomen with sequential tension sutures, thereby reducing tension on the pubic area. This helped avoid pulling the pubic mound even tighter during the revision. The postoperative photos taken one year after the revision show a much lower, more symmetrical scar.
