This patient in developed extensive skin laxity after weight loss from 317 down to 158 pounds while using a GLP-1 medication. She also had a post-partum diastasis recti (separation of the abdominal muscles) which further contributed to the bulging appearance of the abdomen. A fleur-de-lys abdominoplasty was performed to remove a very large amount of excess skin and fatty tissue from the abdomen, which also included removing excess tissue from the mons pubis. A rectus muscle plication was performed, which was a two-layer plication of the entire diastasis extending from the xiphoid process to the pubic symphysis (ribs to pubic bone). The following year we did minor additional liposuction of the pubic mound under local anaesthetic to further improve the final contour. Since this lovely patient has some underlying health issues the procedure was done on an outpatient basis at Southlake Hospital. However, patients in very good health will often have a fleur-de-lys tummy tuck at our clinic, which is a more affordable option. The main advantage of the fleur de lys tummy tuck is that much more skin is removed. Disadvantages include more scars, longer surgical time so higher cost, and slightly higher risk of complications than a regular tummy tuck. The postoperative photo was taken 1 year after surgery.
