This healthcare professional in her 40s developed postpartum sagging and loss of upper breast fullness, known as “involutional ptosis”. She wanted a moderate increase in breast size, in keeping with her body proportions. During surgery, the augmentation was performed first, by inserting 300cc smooth round cohesive silicone gel implants in a subpectoral plane. This type of pocket provides more tissue coverage over the implants and decreases the risk of rippling. After the implants were inserted, a customized breast lift was performed. The anchor scar lift permits removal more excess skin than the other types of mastopexy operations, so it is helpful for rehaping the breast in cases of advanced breast sagging. Very advanced sagging is called “stage III ptosis”.
